At our Shibuya Medical Association, primary care dementia physicians are available to give appropriate advice to people who are concerned about dementia or suffering from dementia. Furthermore, in cooperation with the Community Comprehensive Support Center, we also have doctors who provide dementia consultation services.
Primary Care Dementia Physicians
Shibuya Medical Association certifies members who have completed the prescribed training on dementia (dementia support physicians and family physicians who have completed training on dementia) as <Primary Care Dementia Physicians>. If you have questions or concerns regarding dementia or long-term care insurance, please see your local primary care dementia physician.

About the Shibuya Ward Dementia Consultation Project
We dispatch doctors to the four Enhanced Community Comprehensive Support Centers in the ward (Tsubame-no-Sato/ Honmachi-Higashi Center, Ayame-no-En/ Yoyogi Center, Care Community/ Harajuku-no-Oka Center and Toyosawa/ Shimbashi Center) to provide general consultations on dementia and support as part of the Dementia Early Intensive Support Team. For consultations, please contact your local Enhanced Community Comprehensive Support Center. Consultations are free of charge.
Tokyo Metropolitan Community Collaborative Medical Center for Dementia
The Dementia Medical Center provides specialized medical consultations, diagnoses dementia and plans the initial response, deals with physical complications and behavioral and psychological symptoms, and provides support for people with dementia and their caregivers. For consultations or medical examinations, please call us first.
